I hereby attach an official letter of certification to qualify the aforesaid person for the Fellow / Paramedic / Allied Health / Technician / Student Registration fee.

Conditions for Payment by Local Cheque, Bank Draft and Telegraphic Transfer:

1. All local cheques, bank drafts and telegraphic transfers should be made payable to “National Heart Centre of Singapore Pte Ltd” in Singapore Dollars (SGD) at the amount shown on the reverse page.
2. Please indicate the registrant’s name on the back of the local cheque or bank draft and send it to the Congress Secretariat.
3. Please indicate the registrant’s name on the front of the telegraphic transfer advice after payment has been made and either email or fax the advice to the Congress Secretariat.

4. Payment made via Local Cheque or Telegraphic Transfer must reach the Congress Secretariat not later than
   15 September 2009, Tuesday.
5. Payment made via Bank Draft must reach the Congress Secretariat not later than 18 August 2009, Tuesday.

IV. TERMS & CONDITIONS

• How to Register
  • All participants are encouraged to visit the www.isrbp2009.org for individual online registration which opens on 1 April 2009.
  • Alternatively, you may complete and sign the PDF registration form and send it back, with the appropriate payment details and all related correspondence properly enclosed, to the Congress Secretariat at the address stated above. Kindly note that registration forms received without payment or credit card authorisation will not be processed. Only individual registrations will be accepted (one registration form per participant).
  • If you have already registered online or faxed your registration form with all payment details, please do not mail a copy of this form as it may result in a duplicate registration.

• Registration Fee
  All participants are required to pay the Registration Fee (in Singapore Dollars) at the amount shown on Section II of this form.
  • Full Delegate (Member / Non-Member / Fellow / Paramedic / Allied Health / Technician / Student) is entitled to:
    • Name badge, congress bag, programme and abstract book
    • Admission to all scientific sessions at the 17th Congress of the International Society for Rotary Blood Pumps (ISRBP) and trade exhibition
    • Invitation to opening ceremony
    • Two tea breaks and one lunch per full-day conference
    • Certificate of Attendance
    • Full delegates are entitled to one ticket to Congress Banquet and Futsal Tournament (except for Paramedic / Allied Health / Technician / Student)
    • Paramedic / Allied Health / Technician / Student are entitled to one ticket to Congress Banquet. Tickets to Futsal Tournament are chargeable.
• Additional Tickets
  • Congress Banquet
    • Congress Banquet is chargeable at SGD150.00 per person plus 7% GST or prevailing tax at time of payment.
  • FUTSAL Tournament (Limited tickets only, First-come, first-served basis)
    Delegates who wish to participate in this tournament will each be charged an additional fee of SGD120.00 plus 7% GST or prevailing tax at time of payment. Tournament participant will also be entitled to the following:
    • Dinner for 1 person on day of FUTSAL Tournament
    • 2-way Transfer from Congress Venue to tournament grounds and back

• Registration Deadlines
  
  • Deadline for Early Bird Registration : 31 July 2009, Friday
  • Deadline for Standard Registration : 21 September 2009, Monday

• Confirmation of Registration
  Once payment has been cleared, you will receive the confirmation either via email or fax only. Please include your email address and fax number clearly in the registration form.
  
  To expedite the collection process onsite, please bring along your registration confirmation letter to the Registration Counters (located at the Ballroom Foyer, Level 3, Furama Riverfront Singapore) as proof of your payment and registration.

• Cancellations and Refunds
  Any cancellations and request for refunds must be submitted in writing to the “17th Congress of the International Society for Rotary Blood Pumps (ISRBP)” Congress Secretariat by 15 August 2009, Saturday. Refunds, less SGD100.00 administrative fee, will be made within two months after the 17th Congress of the International Society for Rotary Blood Pumps (ISRBP). No refunds will be made for requests received after 15 August 2009, Saturday.

• Occupation
  For Fellow / Paramedic / Allied Health / Technician / Student / Member, you may be required to provide necessary proof and/or membership onsite when requested by the Organiser or Congrees Secretariat.

• On-Site Registration For on-site registration, only cash (Singapore Dollars) and credit card payments will be accepted. The registration, tour and information counters will be located at the Ballroom Foyer, Level 3, Furama Riverfront Singapore.
